OCI Card for (Adult/Minor who earlier held Indian passport).

both the parts - Part A& B should be signed. (Application Form

of Renunciation of Indian Citizenship.

Two

For
minors (additional documents):

Indian origin is claimed as basis for registration as OCI
cardholder.

Attested
Birth Certificate and Marriage Certificate (as mentioned in the
General Information) where the names of both the parents are
mentioned.

of City Registration/Address Proof.
For
Minors (additional documents):

A
copy of the current passports and resident permits of parents.

certificate of the applicant (attested/apostilled as mentioned in

Marriage
certificate (English translation). Marriage should have subsisted
for at least two years at the time of submission of application for
OCI registration in respect of the foreign spouse

Marriage
certificate issued by any Indian authority: Marriage certificate
must be attested/apostilled by CPV Division, Ministry of External
Affairs Govt. of India, New Delhi.

certificate issued by Malawian authority Marriage certificate must
be appostiled by the Ministry of Foreign Affairs.

Marriage
certificate issued by any third country authority :Marriage
certificate must be attested by Ministry of Foreign Affairs of the

Affidavit/Self declaration of the effect that the Indian/OCI spouse
is still alive and that the couple are not separated/divorced

Indian nationality etc.)

Note:
Foreign spouse OCI applications can only be
processed by appointment as the applicant and the spouse of the
applicant's personal appearance before the Consular Officer is
mandatory. It is necessary to bring all original documents to the
appointment.

case of lost OCI card, police report, in original for the loss of
OCI card containing the lost OCI card number.

OCI card in lieu of PIO card:

filled in and signed wherever necessary.

The
original PIO card can be submitted to the High Commission at the
time of application submission or at the time of collection of the
OCI card. The PIO card will b retained by the High Commission.
